                            First of all good luck with your 2nd rehab....your attitude like always is great. Interesting on the cartilage issue. I had an MRI on my knee about 2 months ago ...which would have been 11 months post quad surgery and I would guess if I had cartliage damage it would have shown up then....but the MRI said the knee was intact. But....still an interesting thought as now....13 months post op I can still not stride run at all. Have seen a second surgeon ..as I reported on here earlier and he said full strength will still take time.....but I am still not sure I will ever run again normally. I should be happy I am pain free.....have full ROM....can bike and do any cross training....but having so much road running and racing in my background it is not the same.....as an old runner like you can understand. I had a gait analysis done by a professional and found that post op my hip on my quad surgery side was 11/2 inch higher than the other side. Fitting me in minimalist running shoes and the barefoot type running shoe ( Vibram 5 fingers) brought me back in balance. This has given me some better gait balance but is not a cureall. Interesting....how a surgery can affect and throw off other body mechanics. I guess we are never quite the same post op...no matter what they say. Keep up your rehab...I will be thinking of you for the best.

Hi Tom, aka, Old Runner Guy,


A good friend who I used to do long runs with is 9 years old than me and was a very accomplished runner and triathlete.  To put things in perspective he ran the NYC marathon in 2:34 and won the 40+ division of Iron Man.  Anyway, Guy, a South African used to tell me that we are all allotted a certain amount of miles in life and what matters is how we space them out over our lifetimes.  What my surgeon told me yesterday was in line with Guy's comment.  He said I can go hog-wild into running again and have problems later or cross train and extend the useful life of my knees.  He suggested running twice a week and cross training the rest of time but at the end of the day it's my decision.


Anyway, this is a reality check.  We are approximately the same age and both have a passion for running.  I have to accept my plight as part of the aging process and move to the next phase.  I can still use my road bike, all the machines in the gym, walk and do limited running.  I just can't compete in road races and go for PRs every weekend.  Things could certainly be a lot worse!


BTW, the pain subsided overnight and I only took one does of percocet so far.  I really try to avoid pain meds unless it is an emergency and last night it was!

If you are using the cross trainer then try going backwards as well as forwards as there is evidence that this can assist recovery in knee injuries.

Yes, it is a shift in direction.  We all have to adapt and adjust as we go through life. Things could be a lot worse.  I will be able to do everything just more of some things and less of others such as running.  I just have to be prudent and get the most milage possible out of my knees.  The last thing I want to do is use up my cartilage and face knee replacements later in life.  I want to take what I have to the finish line!

Here's an update:


Me: 42yo guy, active with skiing, cycling etc.

Injury: 07/25/2012 L complex partial quad tear jumping on a trampoline

Surgery: 07/30/2012


I'm at 18 weeks postop, just had an Ortho appt, and the PT sessions are continuing. ROM is 135 degrees "cold", and about 142 after the PT stretches it. Continuing strengthening exercises, but still clearly weak on the operated side. Going down stairs is still a challenge - I still hold the rail for support. We just started a "walk to jog" progression, and the first few steps jogging were very iffy - it felt very unstable and weak. It's clear there's still a long way to go getting the strength back to normal, but looking back, there's definitely been lots of progress.

                                               Your words are becoming so true to me....I told my wife yesterday at least when I got this injury it was later in life and my running career. I have had the satisfaction of doing 36 marathons..including Boston 3 times and many half marathons and other road races. Heck....most of my wardrobe at one time was running related finishing shirts. Like you....I can still bike, cross train on the machines, and walk. So....be gratefull for what we have and can do. Good luck on that new rehab....think of you often.

Here I join all of you...I tapped in on 11/30/12 after a slip and fall on wet concrete with our dog at the beach.  Pretty much the same with the L knee folding under me and knee cap dislocating medial.  I am a fire captain/paramedic of 40 years in southern california, now 57 yo, extremely active.  Documented full  quadracept/tendon tear.  Repair on 12/11/2012  Looking forward to additional help with rehab...and I don't follow rules well.  Your experience will guide me...going back to Idaho Guy.  Locating  a pool as I write.  Brian

I'm 77 yrs old - active Carpenter. Just about 13 weeks post-op. Working on my first floor tile job since surgery. A little slow but each time I get back to a normal activity I get a boost in spirits and healing. PT today got me to do a full rev on exercise bike - about 70 revs in total. Now at 110 degrees ROM. Will see Doc next week - he wants me to be 120 degrees ROM when I see him. Keep on pushing.......

I would like to know if anybody else is experiencing this, but over the last couple of months, as i wake in the morning, I've been getting what i can only call leg tremors in the leg i had the QTR repair. To begin with they were quite mild and didn't bother me too much, but over time they have been getting worse to the point where over the last week or so they have been quite violent and it's been waking me up in my sleep, This morning in particular was very bad and i found myself stretching my legs as i was waking, this caused me to get searing pain just above the knee so painful i had to stop myself from screaming. I have an appointment with my surgeon again next Tuesday and I'm going to insist on having an MRI, I'm also going to see my step father tomorrow who is a former consultant anaesthetist to ask his advice on the whole matter, though he may not be familiar with my specific injury, he has certainly worked with enough surgeons in his working life to be able to advise me on how to deal with this surgeon who doesn't seem to listening to my concerns.


Going back to my original point, i would like to know if any of you guys have experienced leg tremors of any kind. I have done a bit of research and it could possibly be restless leg syndrome but it's a real pain in the leg....excuse the pun.

I used to get muscle spasms after lying down for a while. This would wake me up in the middle of the night with no relief. I asked my ortho Dr what to do. He prescribed vallium for the spasms and it helped me get back to sleeping.

Yup, I get tremors (full quad tear surgery on Aug 16, 2012) but not like the ones you describe. Nothing's the same physically. For the short time between the accident (fell down stairs) and the surgery I was crippled. Couldn't extend my leg at all. Surgery fixed the mechanics of attaching muscle (most of it, some was lost because it retracted) to tendon but threw all sorts of other things "out of joint". But...I can walk and stand without my leg buckling. I found that aggressive physical therapy 2-3x weekly to the surgical area and hamstring combined with the recumbent bike has helped greatly to break though scar tissue and improve my ROM. I'm nearing 120 and want to keep going.


I tried navigating the crowds in NY City on a rainy evening and really got a wake-up call. My open field walking isn't working and I can't cut and dodge nor speed up or slow my gait quickly in pedestrian traffic. Walking on the wet surface slowed me even more. It's getting colder now and the rain will turn to snow and ice. I bought the metal chains for my shoes; still, not looking forward to learning how to walk again on ice post-surgery.


Does anyone know what type of rubber sole (Vibram?) grips frozen surfaces best?

Your documentations and replies have been very helpful and appreicated.


I will be reading here a lot over the next year....the experiences and advice will be very helpful to us.


My husband 56, very active and in great shape had a quad rupture while on the construction job last week.

 Surgery was yesterday and it went well. Today is his second day at home and the nerve block is wearing off, so I am trying to keep ahead  with the pain management.


This will be a great resource when he starts his rehab.

This injury sure has a long process of healing! I know he is going to get restless very fast.


I look forward to reading what worked for those of you who already experienced this.

Once he is up to it, I will have him reading your  documentation and all the replies.

That is too bad you were in so much pain after your surgery.

If you are reading this and are waiting for your surgery - my husband had his surgery yesrerday ( 12/7/12) and we fairly comfortalble the first 24 hours considering.

He received a frontal block so his whole leg had no feeling the entire day or night. 


So, request a frontal block if you can. They give you an ultra sound and look for the nerve up near your hip and insert the medication.

About 24 hours later it wore off and we got vigalent on the pain meds. He has been resting pretty comfortably all day, however is

using the urinal as it is painful to get up and walk. He is pretty  much stuck in his recliner not able to get up, but keeping the  pain at bay.

Doing his in chair excercises and waiting for this first day post surgery to pass.

As a wife's perspective...this isn't a great post op experience.

Nurse maid is keeping busy.

I had a complete rupture and avulsion 21 months ago.   I'm 51 years old and very active.  


The one thing my gal did for me was keep me honest and conservative.   I wanted to push rehab and she would say no.

I wanted to reject the surgeon's directions and she would say no.   She was the primary reason I recovered 100%.


It will get rough....no doubt.

I had intermittent leg tremors waking me from sleep but not as painful as you described and fortunately they did not last very long, they stopped about 4 weeks ago and I have not had them since. Vit B complex particularly B12 can  help with muscle spasms so this might be worth a try before benzodiazepines like Valium.  I hope your quest to get an MRI is successful, at least it should pick up an additional pathology in your knee, cannot do any harm and if you are considering additional surgery I would have thought this non invasive investigation would be mandatory.  

My physio has recommended that I do individual leg presses on the machine at the gym because my good leg was compensating too much for the weak one doing squats. I tried this yesterday and can feel the additional strength in my leg walking up stairs so I am planning to do some more. I am continuing with reverse cycle on the X trainer, the horizontal cycle and walking in the spa with gentle continuous stretching of the tendon in the warm water. Like most people blogging here I have concluded its a long slow process.

Well, my fearless prediction that surgery to repair a fractured kneecap would be a walk in the park compared to my original quad tendon rupture was not on target.  On a scale of 1 to 10 the quad injury was a 10 but I have to say the kneecap repair and post op distress was easily an 8.  I strongly recommend you not walk in my kneecap park!


Anyway, I changed the dressing on my knee last night and saw the surgeon's handiwork for the first time.  I've got a zipper knee from the bottom of the kneecap up into my quad.  The wound looks "good" so I think the healing process is going well.  Psychologically looking at the knee was hard to take.  See the surgeon again on Friday.


I hope your rehab efforts are progressing and you are soon land running again!  Keep me posted on developments.

Hi all, been following the log for the past year, thought it was time to actually log in so here goes. Fell hard on my concrete driveway on 4/12, torn the right quad tendon cleanly off of the knee cap. After 2 weeks of missed diagnosis, MRI indicated a complete tear along with a chipped knee cap. Surgery on 5/2 with 4 nylon sutures to attach quad muscle in place. Dr. was a Mayo Clinic graduate so 3 months of limited movement, though I did do weight bearing early and early walking with crutches. After 3 months started the PT along with therapy for ROM, which was slow, was able to drive with a 90 deg. after one week of PT. At 8 months I can walk with a slight limp, both knees still sore after a walk, never had any problems with swelling or the need for ice. Was a runner for many years but at 71 yrs. the knees were sore before the accident, tried a slow jog the other day but very unstable and unsure if I'm ready yet for that test. Can do stairs up and down but always with the railing for the down hill steps, leg is still not the same as the other one and is weak after an hour walk. My ROM is between 100-115, and i can live with that, doing double up steps for strength but the numbers don't move much. Really don't like the idea of another procedure as far as scar tissue or forced ROM movement, imho. as everyone has said this is a long journey and those with the total knee replacement seem to be looking back at us and wonder whats the problem. Not sure if my injured knee will ever be quite the same, where walking down a hill or stairs hands free, but there isn't a day that goes by that I'm not aware that I have a leg that just is not the same as the other, maybe a little more time will change that. With that said it is quite a journey my friend. Tom

Your history is almost exactily like mine. Age 69; active cyclist, hiker; April 16 injury with accurate diagnosis at small hospital in So Utah (Kanab); April 20 surgery at Mayo Clinic (home); 3 months healing; PT thereafter; have 120 ROM; can do most things I used to, especially cycling, which I actually did before I could walk again; steps painful, but getting better; staying away from running, which with cronic bad knees I haven't done often for years; still have scar tissue I can feel and hear when I bend leg.

I hope your and my good outcomes, despite our age!, will speak about letting the healing take place before any aggressive PT. Patience is the mantra. You will get better.

I had another appointment with my surgeon today and although I'm still not getting an MRI, he has agreed to do an arthroscopic scar tissue removal and manipulation. Hopefully this will give me the opportunity to get a bit more ROM, just have to wait for the appointment to come through now.

Hello Boston, NYYankee (~8 week guys) et al-


Update - LQTR 10/6, 10/16 repair.  At 8 weeks PO I'm at 60 degree's on the brace and walking fairly comfortably.  Using a cane and commuting into NYC.  I'm getting just past 90 degree in PT and starting to use the  exercise bike (not even close to a full rotation.)  Working on getting into a pool soon.  Question - I still have fairly significant swelling in the knee,  and now have  "pitting edema" (per PT) in my calf, despite regular use of the iceman.  I'm getting a compression stocking.  Would appreciate color on how long others have had swelling post op and if anyone else had it spread down the leg.

Just a thought on edema, I was having trouble with it pt was advising compression socks and manual manipulation @ $100/hr so I got a second opinion.  They recommended aspirin(asa), huge increase in fluids and cut way down on salt.  Noticable difference in 3 days. Also try to change contact point of desk chair edge and hamstring.  Probably would have gone with the socks but up here in this podunk town everything is a week or 10 days out.  Good luck and good balance.

I have been reading this thread for weeks now as a source of information and support so now I will weigh in as well.


I completely tore my right quad tendon Sept 21 2012 during a wrestling match.  I had it surgically repaired the next day.  Yesterday was eight weeks since the surgery.  


I was told by the surgeon to keep the brace on for at least eight weeks and use crutches with no weight bearing for at least six weeks.  I had agreed to listen to all instructions as the post operation pain was very severe and because of my size (6'6" and over 300 pounds) I thought it made sense.  However, within a few days I was having trouble using crutches and only used them for getting up and down or outside.....they were discarded at the two week mark when i decided to go back to the gym for upper body training.  I did however wear the brace 24 hours a day until about the five and a half week mark. I was very nervous without the brace but I was careful and had already started my pt at this point.  ROM has been an issue as it took two weeks to get to 90 degreees and as of this week I am at about 100.  The muscle size is already starting to come back and I think it's due to the fact that i do hours of physio on my own per day like stetching, heat, body weight squats and all of the basic things they tell us to do.  I even did some leg presses at the gym yesterday although with a light weight.


My only issue seems to be my walking.  It still feels very tight and I limp much worse than i had expected to at this point.  Any feedback on this would be appreciated.

My full avulsion rupture was over a year and a half ago.    We are similar in weight but not height.   I'm a very active 51 yr old powerlifter.  


I was much more conservative with weight bearing.   I went as far as getting a wheel chair for 8 weeks.   And, I approached rehab cautiously as well.  


As for the gait, I walk many meters in the pool.   It allowed me to walk perfectly without a limp.    And your ROM is outstanding at this point.    I would just recommend


patience.   I'm pulling 295kgs in meets now.    All good.

     About your limp, walk then walk some more, swing your arms and move your hips like a 17 year old girl.  I know it sounds stupid but its working for me, 59 yrs old 6'4" 325 lbs.  I never have been much to look at so why worry?(17 months after left quad,right pattelar)  Good luck.

I totally agree with bigggfred, your ROM is extremely good after only 8 weeks, i myself am at 98 degrees after 27 weeks and have been for 3 months now, so I wouldn't worry too much just yet.
As many on here have said patience really is the key and bear in mind this is one of the worst leg injuries you can sustain, the road to recovery from which is a long and slow process so hang on in there my friend, you will get there.

Thanks to all of you for your feedback and advice.  I am going to the pool later today to practice my walking.  I go back to physio tomorrow expecting to be at 105 ROM or more.  I gave myself a full day of rest yesterday other than an arm workout.


I know I am more aggressive than most but I am being careful and i have the benefit of the knowledge that several pro wrestlers have come back from this injury as I intend to in May which will be seven months post op.  I also have the experience of having ruptured my biceps tendon and pectoral tendon so I recognize different feelings associated with healing.


I also have a friend in England who was too laid back in his quad rupture rehab and he is still on crutches after three years and has no chance of recovery now.  Human beings are designed to work but we are also designed to think so i am trying to do both.

Hello fellow 8 weekers,


I have been deemed by the OS and PT as a "sweller".  My knee is still pretty well swollen despite icing during the day and wearing my compression sock most of the day.  OS said normal for some people. 


I was fortunate to get to 90 degrees fairly quick but went about three PT sessions with little improvement.  Started doing extra stretching during breaks at work and the past two sessions I made 115 degrees true measure.  Last meeting with OS was 12/14 and he recommended walking around home/work, basically controlled enviornments without the brace.  Outside of that brace is set at 75 degrees for protection.  PT consisting now of 15 minute warmup on the bicycle (felt great to finally be able to ride and "exercise"), leg raises, standing hamstring curls, calf raises, one leg press on a device that looks like a sled, electric stim as usual, and still emphasis on the quad squeezes as getting that knee to slide up and down properly is crucial.  I was disappointed while stuck around 90 but feeling confident about the progress...I know we have a long way to go.  Focused on regaining that ROM, I am used to working out 5-6 days a week, the strength will come down the road.  Interested to hear others PT routines and hopefully positive ongoing recovery.                    NYYANKEE
