Downsides to this method of boot fitting?

This is a method i've heard advocated by several bootfitters:

1. Size for the smallest part of your foot (usually the ankle), Ignore all other pain, that can be dealt with later.
2. Take a dremel to that sucker and grind out each hot spot till the shell is as molded to your foot as your liners are.

Is there a downside to this as opposed to picking a bigger boot, and padding where necessary?

that is the right way

a small boot can be made bigger, but a big boot can not be made smaller.

so you fit for the smallest parts and make the rest bigger
