In NYC, it's tougher. Some people like Jeff Rich; I found him hard to deal with. There is also a Surefoot on 3rd Ave and 68th. The manager there is good, if you like the Surefoot footbed.

See the bootfitter's list for comments (at the bottom) on Jeff Rich. His insoles are excellent but I think now they are about $275. As far as I'm concerned, Surefoot blows (had one for hiking boot remade twice before I cashed it in). I'd much rather have a Superfoot cork made by someone who knows what they're doing (important) than a Surefoot. In nyc I believe Scandinavian on 57th st makes custom Superfeet. (can't comment one way or the other on quality).

Only Jeff Rich. No comparison between Sure Foot NYC and Rich--he is among the best boot fitters there is--he doesn't sell--but if you see him before you buy, he'll make recco's. Its also worht the trip to Stratton to see the guys at Green Mountain Orthotics.
