Boots tight along met heads

A friend feels like the boot is scrunching (for lack of a better term) his foot along the met heads. He had them blown out and he has not solved the problem. He has a custom foot bed. This also happens on the same foot with his cycling shoes.

I have heard of something called a met head spreader. Not sure what it is and how to modify the bed. I understand that this might help spread out the foot and relieve the discomfort. Can anyone provide some guidance here, please?

any decent boot fitter /pedorthist can build.

 

could be the solution, however there are many factors at work inside a ski boot that could be the source of the problem.
