If boot bolt plugs are already in, what do cuff adjustments do?

Hi boot guys,

 

I am wondering what turning the cuff adjustment screws will do on my boots already plugged in all three holes.

 

I am hoping to achieve radial canting, b/c when my boots are on my leg flexes into the 2:00 position on right boot and 10:00 position on left boot.  Therefore the radial axis of the cuff doesn't match my leg and there is nothing else that can be done up there.
