Sounds like you have alignment issues on both sides!  Your tibial issues should be accommodated with the cuff canting and possibly a bit of padding to create equal pressure on each side of the leg.    I would suggest...

I would remove a semi circle of the hard plastic on the tongue to float the pressure around the affected area.  Careful to melt the threads so they do not unravel.  It may not look pretty but remember your boots are tools not...
